Oral Care Should Begin With the First Tooth
Oral hygiene in children should begin as soon as the first primary tooth erupts. The first primary teeth usually begin to erupt around the 6th month, and all primary teeth generally take their place in the mouth by the age of 3. During this period, the teeth can be gently cleaned with a clean gauze pad or a silicone finger toothbrush. It is very important to help children develop a regular toothbrushing habit from an early age. Experts recommend brushing the teeth twice a day, after breakfast and before bedtime, using an age-appropriate amount of fluoride toothpaste. Since hand skills are not yet sufficiently developed in young children, toothbrushing should be performed under parental supervision and with parental support to ensure effective oral hygiene.
Pay Attention to Night Feeding and Baby Bottle Tooth Decay
One of the most important causes of early childhood caries is prolonged night feeding. During sleep, saliva secretion decreases, making the teeth more vulnerable to acid attacks. Therefore, falling asleep with a bottle or while breastfeeding should be avoided whenever possible, and oral cleaning should not be neglected after night feedings. In addition, dipping pacifiers or bottle nipples into sugary foods such as honey, molasses, or jam can significantly increase the risk of severe tooth decay and is not recommended.
Dietary Habits Affect the Risk of Tooth Decay
Children’s dietary habits directly affect the risk of dental caries. Sugary foods and beverages consumed frequently throughout the day, as well as sticky snacks, remain in the mouth for a long time and facilitate the formation of cavities. The consumption of sugary foods should be limited, and the teeth should be brushed as soon as possible after consumption. If brushing is not possible, the mouth can be rinsed with water. Regular consumption of calcium-rich foods such as milk, yogurt, cheese, kefir, eggs, and green leafy vegetables supports the development of teeth and bones in children and also contributes to the protection of oral health.
Harmful Habits Should Be Evaluated Early
Habits such as prolonged pacifier use, thumb sucking, teeth grinding, and mouth breathing can negatively affect jaw development as well as oral health. Dry mouth caused by mouth breathing reduces the natural protective effect of saliva in the mouth and may increase the risk of tooth decay. If these habits continue for a long time, the child should be evaluated by a pediatric dentist or the relevant specialist physician.
Cavities in Primary Teeth Must Be Treated
Delaying or neglecting the treatment of cavities in primary teeth is not the correct approach. Advanced decay can lead to pain, infection, and dental abscesses. In addition, early loss of a primary tooth may cause adjacent teeth to shift into the empty space and disrupt the eruption pattern of permanent teeth. After a clinical examination and, if necessary, radiographic evaluation, the pediatric dentist plans the appropriate treatment according to the extent of the decay.
Composite Filling: If the decay is in the initial or moderate stage, the decayed tissue is removed and the tooth is restored with an aesthetic composite filling material.
Pulpotomy (Partial Root Canal Treatment): When the decay reaches only the living tissue in the crown portion of the tooth, the infected pulp tissue can be removed while preserving the healthy tissue in the roots.
Pulpectomy (Primary Tooth Root Canal Treatment): In cases where the decay has progressed into the root canals, the infected tissues are cleaned and root canal treatment is performed using special filling materials developed for primary teeth that can be physiologically resorbed.
Pediatric Crowns: In primary teeth with extensive tissue loss, stainless steel or aesthetic zirconia crowns can be used to allow the tooth to remain functional in the mouth until its natural exfoliation time.
Why Should Treatment of Primary Teeth Not Be Delayed?
Primary teeth not only enable effective chewing of food but also help prevent future alignment problems by preserving the space where permanent teeth will erupt. The shifting of neighboring teeth into the space created by the early loss of a primary tooth may lead to crowding, impacted teeth, and the need for orthodontic treatment in later years. For this reason, fillings, root canal treatments, and crown applications performed on primary teeth aim to keep the tooth healthy in the mouth until its natural exfoliation time.
Regular Dental Checkups Play a Preventive Role
It is recommended that a child’s first dental examination be performed after the eruption of the first primary tooth and no later than 12 months of age. Subsequent checkups are planned according to the child’s caries risk, but they are generally carried out every six months. Regular dental examinations allow cavities to be detected before they progress, providing easier and more effective treatment opportunities. In children who need them, preventive methods such as fluoride varnish applications and fissure sealants applied to permanent molars can reduce the risk of cavity formation.
